Susten prolonged release vaginal gel 8.0% is indicated for the treatment of absence of a menstrual period in a woman who has not reached menopause. Progesterone is the main ingredient of this medicine. Progesterone is one such female hormone that supports women’s regular menstrual cycle. Inadequate production of Progesterone leads to menstrual irregularities. Susten gel is prescribed to increase the production of Progesterone and resolve menstrual irregularities. It is also included in a treatment program for women who are undergoing fertility treatments to become pregnant. Progesterone is involved in the preparation of the lining of the uterus so it can receive and nourish a fertilized egg and sustain a pregnancy. If you are undergoing IVF or ART, and it is determined by your healthcare provider that you are not producing enough Progesterone on your own. In that case, this medication may be prescribed to increase the progesterone levels.

     ‒    Susten gel should not be used in women if any of the following applies:

     ‒    Known sensitivity to Progesterone or any other ingredients present in the formulation, including any non-medicinal ingredient or components of the medicine.  

     ‒    Known or suspected progesterone-dependent neoplasia

     ‒    Liver dysfunction or disease

     ‒    Undiagnosed vaginal bleeding

     ‒    Missed abortion

     ‒    Malignancy of breast or genital organs

     ‒    Acute porphyria

     ‒    Thromboembolic disorders, thrombophlebitis, cerebral apoplexy or patients with a history of these conditions.   

 

Treatment should be stopped if the results of the liver function test become abnormal or if cholestatic jaundice occurs. Progestogens may give rise to fluid retention. Therefore, women with any health conditions (such as asthma, renal dysfunction, epilepsy, migraine, and cardiac events) affected by this factor require close monitoring.

 

Women with breakthrough bleeding require certain diagnostic tests to determine whether organic diseases are present. Women with a history of depression should require close monitoring by their healthcare professional, and treatment discontinuation is considered if the depression recurs to a serious degree. Although no drug interactions in the case of Susten gel are reported, Susten gel should not be used concurrently with other vaginal treatments. The product is not meant for use in children.

 

Use in pregnancy: The progesterone gel has been successfully assisting embryo transplantation and maintains pregnancies in many cases through its use as a part of the ART  treatment program.

 

Use in lactation: It should be avoided if you breastfeed a baby. A considerable amount of the medicine has been found in the breast milk of women receiving this gel. However, the effect of this on a nursing baby has not been determined.

If you experience any side effects associated with the use of Susten gel 8.0%, please report them to your healthcare professional. These include breast tenderness/breast pain,  irritation of the vagina, urinary tract infection, genital candidiasis, spotting between periods, abdominal pain, headache/migraine, dizziness, vomiting, memory loss, depression, constipation, reactions at the site of the application, vaginal discharge, muscle cramps, bladder infection, itchy genitals, nervousness, accidental loss of urine, dryness of the vulva or vagina.

For reproductive failure and in vitro fertilization (IVF), Susten is used at a dose of 90 mg. The treatment usually started within four days, preferably two days after getting HCG (human chorionic gonadotropin) shots. One application of 90 mg (1.125g, 8%) should be used vaginally daily or twice daily. Most females will respond to a 90 mg daily dose. However, some may require 90mg twice daily dose. If pregnancy happens, treatment may continue for up to 10 to 12 weeks.

 

Instructions for use: The gel is to be applied directly from the specially designed applicator into the vagina. It coats the vaginal mucosa to give a long-lasting release of Progesterone. Each applicator carries a slightly larger amount of the topical medicine than actually released, as the rest of the formulation tends to stick to the inside of the applicator.  Each applicator carries 1.45 g of formulation and is made in such a way that at the time of each administration, an exactly defined amount of the formulation (i.e. 1.125g) is delivered. The remaining product inside the applicator should be discarded.

 

Note: Each applicator is meant to be used for single use only. The dosing instructions given here may be different from the one prescribed to you by your healthcare provider. Therefore, it is always important to use a medicine under medical supervision.

 

Susten 8% gel is administered vaginally. Take out the applicator from the sealed wrapper.

 

Avoid removing the twist-off cap at this stage.

     ‒    Hold the applicator firmly by the thick end. Shakedown, as you would do with a thermometer, to ensure that the medication inside is at the thin end.

     ‒    Twist off the tab

     ‒    Insert the applicator into the vagina while in a sitting position or when lying on the back with your knees bent. The thin end of the applicator should be gently inserted into the vagina.

     ‒    Now, you need to press the thick end of the applicator to release the gel. Take out the applicator and discard it.
