For adults with type II diabetes, Januvia 100 mg is used to control the impact of food on blood sugar levels as monotherapy in patients where the blood sugar is not adequately controlled by diet and exercise alone and for whom another antidiabetic agent like metformin is inappropriate due to intolerance or contraindications.

The medication can be used as a dual treatment in combination with metformin when diet and exercise, or metformin alone, are unable to reduce blood sugar levels.  Januvia can also be taken with sulfonylurea (another diabetes medication) when diet and exercise, plus sulfonylurea alone, fail to reduce blood sugar and when metformin is not appropriate due to contraindications or intolerance.

This antidiabetic medication is used as a triple treatment in addition to metformin and sulfonylurea when dietary restrictions, exercise, and dual treatments with these medicines fail to control blood sugar levels.

This medicine can also be used as an additional treatment to insulin (with or without metformin) when dietary restrictions, exercise, and a stable dose of insulin are unable to control blood sugar levels. Each Januvia tablet contains the active ingredient Sitagliptin (as the main active component), which helps reduce blood sugar levels by increasing the presence of certain substances in the body that cause the pancreas to release more insulin. It also helps to stop the production of sugar when there is an excessive amount of sugar in the bloodstream.

Precautions with Sitagliptin tablet

‒     Patients should not take Januvia 100 mg with type 1 diabetes. It should be used for the treatment of diabetic ketoacidosis (when lack of insulin causes ketones, the harmful substances to build up in the bloodstream.

‒     Dizziness and drowsiness have been reported in those receiving this antidiabetic medicine. This may affect your ability to drive or operate machinery. Taking Sitagliptin along with sulphonylureas or with insulin can contribute to very low blood sugar (hypoglycaemia). Hypoglycaemia may interfere with your ability to drive or use machines.

‒     Your healthcare professional must be aware of your pregnancy status if you are pregnant or planning to get pregnant. The effect of the antidiabetic medicine on breast milk is still unknown. If you are a breastfeeding mother, you should avoid taking this medicine if you are breastfeeding or plan to breastfeed.

‒     Taking other medications during Sitagliptin treatment can interfere with the effectiveness of Sitagliptin or other medications. Be cautious when taking digoxin, a medication used to treat irregular heartbeats and other heart conditions. If this is the case, your healthcare specialist will check digoxin levels in your body.

Side effects of Sitagliptin tablet

Episodes of hypoglycaemia (low blood sugar) have been observed when used with sulphonylurea and insulin. Discontinue treatment and seek medical attention right away if you develop serious side effects, including:

‒    Pancreatitis (inflammation of the pancreas) have been observed. Severe and persistent pain in the stomach area, which might travel to your back, too, with or without nausea and vomiting, as these could be signs of an inflamed pancreas.

‒    Hypersensitivity reactions are serious allergic reactions, which include hives, rash, blisters, swelling of the face, lips, tongue, and throat, as well as blisters and peeling skin. In such cases, a healthcare specialist may recommend medications to treat your allergic reaction and a separate medication for diabetes.

Other reported symptoms include headache, dizziness, interstitial lung disease, vomiting, constipation, rash, pruritus, angioedema, urticaria, myalgia, back pain, flu-like symptoms, dry mouth, stuffy nose, upper respiratory tract infections, sore throat, reduced platelet count, and impaired kidney function.

Some people develop different types of abdominal discomfort when starting the combination of Januvia and metformin.

If you experience any side effects that become bothersome or don’t resolve within a few days, consult your healthcare provider.

How to take a Sitagliptin tablet?

Typically, Januvia is recommended at a 100 mg once-daily dose, either as monotherapy or in combination with metformin, metformin and a sulfonylurea, insulin (with or without metformin), or pioglitazone (with or without metformin). Continue using this medicine as prescribed to help control your blood sugar levels.

If a dose is missed, take it as soon as you remember. If you forget to take it until it is time for your next dose, skip the missed dose and follow your regular dosing regimen. Do not take two doses on the same day unless your healthcare professional instructs you to do so.
